Home Connections Sheet
This sheet has “jumping-off-point” questions for your quarterly book talk with an adult.

Reading Questionnaire
This is the sheet you fill in each quarter about your reading for that quarter and your reading plans for next quarter.  You will attach your summary of your book talk with an adult.

Personal Reading Log
This is the sheet where you keep track of the books you read, how many pages you read, and how much you enjoyed the books.
