Health Forms

If you did not do a fall sport, the online health packet needs to be filled out.  To begin, a new account for the rSchoolToday will need to be created.

Physicals need to scanned in or you can take a picture of the physical and upload that.

If you did a Fall sport, this year, you should already have an athletic packet on file and you need to transfer your paperwork to Indoor Track. Just send an email to Ms. Maines in the office.
